Jim Folsom, MA, LPC, LISAC, Clinical Director
Jim received his Bachelor's degree in psychology from Northern Arizona University in 1979 with Summa Cum Laude honors and received his Master's in Guidance and Counseling in 1980 from NAU. Jim has been working in the field of chemical dependency and mental health since 1979. He has an extensive background in working with people who grew up in dysfunctional families, treating people with co-occurring substance abuse and mental health diagnoses including depression, anxiety, panic attacks, obsessive compulsive disorder, post traumatic stress disorders, grief and loss, stress and anger. Jim also does family interventions to help the resistant alcoholic/addict into treatment. Jim has been facilitating Chemical Dependency Intensive Outpatient Programs (CD-IOP) since 1987.
Hazel M. Tate, R.N. Nurse Practitioner
Hazel has worked in behavioral health for 14 years. She is also experienced in crisis assessment, medication management, addictions, dual diagnosis and outpatient detox assessments. Previously she was in an independent practice in family medicine and school health. She is also skilled in research of psychiatric medication and other drug interactions.
Keady Maudsley , MA , LPC
Keady has 25 years experience in working with mental health and substance abuse issues. She has a master's degree in counseling from California State University and a year's additional training in chemical dependency from UCLA. Keady is licensed in both Arizona and California. Keady works with adults, addressing issues of depression, anxiety, relationship difficulties, grief and loss, stress, anger and low self esteem. She also works with substance abuse, addictions, and family related issues of co-dependency and alcoholic families.
Tracy Landrus, MA, LPC
Tracy is a licensed professional counselor in the Phoenix area. She received her Bachelor's degree in social work from Southeast Missouri State University in 1987. Her Master's degree in counseling services is from Webster University, and was received in 1990. She has 14 years experience in working with mental health issues in both in and outpatient settings. Tracy works with children, adolescents, and adults on issues such as depression, anxiety, adjustment disorders, self esteem, self injurious behavior, parenting issues and stress management. Her areas of expertise include crisis management and adolescent treatment.
